We do have specific appointments that are scheduled for a consultation only for the following circumstances.

     
 
  • Diagnostic consultation
  • Apicoectomy (root canal surgery)
  • Re-treatment (a previously treated root canal that may need to be re-treated)
  • Exam only for patients who would like to go over treatment options.
  • I.V. Sedation (patients who may be considering I.V. sedation)
 
     

Please assist us by providing the following information at the time of your initial visit.

     
 
  • Your referral slip and any X-rays if applicable
  • A list of medications you are presently taking
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
  • If you are a patient who would normally take antibiotic premedication for dental treatment you may be required to premedicate prior to your consultation, so our doctors can complete a thorough exam/probing of the area in question.  Please inform our receptionist at the time of scheduling if this is the case.
 
     

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or guardian at the initial visit.  The parent who brings the minor patient in for treatment is the parent who is financially responsible for treatment.

Please alert the office if you have a medical condition that may be of concern (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)

X-Rays

If your dentist or physician has taken x-rays, you may request that they forward them to our office.  These x-rays/images can be used in addition to the x-rays/images we will take in our office during the course of treatment.  We always take our own images for our records regardless of the x-ray/images your dentist may send.  If this is a concern to you, please call our office at (209) 478-2252.
